Responsibilities

- Foster understanding of clients' values and assist them in aligning their actions accordingly.

- Develop and maintain relationships with clients, their families, colleagues, and other teams to support clients in achieving their goals.

- Implement tailored activities to enhance client independence and encourage self-sufficiency.

- Celebrate clients' achievements and acknowledge their progress.

- Connect clients with community resources as desired, promoting access to necessary support systems.

- Document clients' requirements and reinforce positive behavior.

- Maintain professional responsibilities such as mandatory reporting, legal obligations, contract KPIs, and documentation.
