Aged Care Needs Specialist

This is a permanent full-time role for an Aged Care Needs Assessor to join our dynamic team at Northern Sydney Local Health District.
Key Responsibilities:
- Undertake comprehensive assessments of older people's needs and develop appropriate care plans.
- Work collaboratively with Occupational Therapists, Physiotherapists, Social Workers, and Registered Nurses to provide high-quality aged care services.
- Contribute to the development of education programs and resources for staff and clients.
- Demonstrate strong written and verbal communication skills and use technology effectively to manage patient records and communicate with stakeholders.
Requirements:
- Relevant tertiary qualifications and full registration or membership of the relevant Australian peak professional body.
- Strong discipline in clinical experience working in aged care settings and proven ability to undertake comprehensive assessments and formulate care plans.
- Detailed knowledge of My Aged Care, related aged care programs, and services with proven ability to use this knowledge to assess and support aged care presentations.
