Aged Care Needs Assessment Opportunity

We are seeking a highly skilled and compassionate Aged Care Needs Assessor to join our dynamic team in the Northern Sydney Local Health District.

The role involves providing aged care assessment services across various sites, working collaboratively with Occupational Therapists, Physiotherapists, Social Workers, and Registered Nurses to deliver high-quality patient care.

To be successful in this position, you will require excellent education and clinical supervision. You must also possess a strong passion for working with older persons and a commitment to delivering exceptional patient outcomes.

Key Responsibilities:

  • Conduct comprehensive aged care assessments to identify individualized care needs
  • Develop and implement effective care plans in collaboration with multidisciplinary teams
  • Maintain accurate records and reports, ensuring compliance with regulatory requirements
  • Participate in quality improvement initiatives, contributing to continuous service excellence

Requirements:

  • Relevant qualifications and certifications in aged care or a related field
  • Proven experience in aged care assessment and planning
  • Excellent communication and interpersonal skills
