**Job Overview**: We are seeking a passionate and dynamic NDIS Support Coordinator to join our team at Infinity Coordination & Supports Pty Ltd.

 

**About the Role:** As an NDIS Support Coordinator, you will play a crucial role in empowering participants to achieve their goals. Your responsibilities will include identifying service options, coordinating supports, and strengthening participants' abilities to self-direct their NDIS plans.

 

**Key Responsibilities:

  • Assist participants to implement their NDIS plans, explore options for services/providers, and coordinate services that match their unique needs.
  • Transform participants into community explorers, strengthening their abilities to self-direct their supports and dive headfirst into a world of exciting opportunities.
  • Develop meaningful relationships with participants and their families while maintaining professional boundaries.
  • Monitor plan outcomes and spending to ensure every dollar is put to good use, ensuring that participants receive the best services within budget.
  • Hold mainstream and community services accountable for fulfilling their obligations to the participant, from housing to health.
  • Keep things organized and efficient with clear and accurate records of information, time, and billing.
  • Build rock-solid relationships with stakeholders and agencies to enhance our services and meet the diverse needs of our clients.
  • Help participants develop resilience and conquer adversity.

 

**Requirements:** To be considered for this role, you will need to hold (or be willing to obtain) the following qualifications and certifications:

  • NDIS Worker Screening clearance.
  • Fully Vaccinated for COVID-19.
  • Hold a valid driver's license and have access to a reliable car with comprehensive motor vehicle insurance.
  • Cert in Disability, Community services, Nursing, or equivalent (preferred).
  • Working with Children's Check.
