Seeking a dynamic and compassionate NDIS Support Coordinator to join our team at Infinity Coordination & Supports Pty Ltd. If you are passionate about delivering person-centered supports and empowering individuals with disabilities to achieve their goals, this role is for you.
About the Role
We are looking for a highly motivated and experienced Support Coordinator to work closely with participants in Albury/Wodonga, Northeast Victoria, and Goulburn Valley regions. As a Support Coordinator, you will play a vital role in helping participants navigate the National Disability Insurance Scheme (NDIS) and achieve their desired outcomes.
Key Responsibilities
- Empower participants to implement their NDIS plans, explore service options, and coordinate services that meet their unique needs.
- Transform participants into community explorers by strengthening their abilities to self-direct their supports and access new opportunities.
- BUILD MEANINGFUL CONNECTIONS: Develop lasting relationships with participants and their families while maintaining professional boundaries.
- Monitor plan outcomes and spending to ensure every dollar is utilized effectively, providing participants with the best possible services within budget.
- HOLD MAINSTREAM AND COMMUNITY SERVICES ACCOUNTABLE: Ensure stakeholders fulfill their obligations to participants, from housing to health.
- Record-keeping expert: Maintain organized and efficient records of information, time, and billing.
- STAKEHOLDER COLLABORATOR: Foster strong relationships with agencies and stakeholders to enhance our services and meet diverse client needs.
- Crisis Resolution Specialist: Provide guidance and support during challenging times, helping participants develop resilience and overcome adversity.
Requirements
- National Disability Insurance Scheme (NDIS) Worker Screening clearance.
- Fully vaccinated for COVID-19.
- A valid driver's license and reliable car with comprehensive motor vehicle insurance.
- Certification in Disability, Community Services, Nursing, or equivalent (preferred).
- Working with Children's Check.
