Job Title: Case Manager

  • Travel to various locations within the health district.
  • Work as part of a dynamic multidisciplinary team in a rural setting.
  • Deliver contemporary brain injury rehabilitation services to adults with acquired brain injuries.
Key Responsibilities:
  • Ensure delivery of high-quality rehabilitation services that meet the principles and guidelines of the NSW BIRP.
  • Collaborate with other healthcare professionals to provide comprehensive care to clients.
  • Develop and implement individualized rehabilitation plans that address the unique needs of each client.
  • Monitor client progress and adjust treatment plans accordingly.
Required Skills and Qualifications:
  • Relevant degree or qualification in a field such as occupational therapy, physiotherapy, social work, or psychology.
  • Proven experience working in a healthcare setting, preferably in a rural or regional area.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
