Job Opportunity

We are seeking a skilled professional to join our multidisciplinary team in the Aged, Disability & Palliative Division. The successful candidate will be responsible for delivering contemporary brain injury rehabilitation services to adults with acquired brain injuries in the Western NSW Local Health District.

About the Role

The Case Manager will work closely with a team of senior clinicians to ensure the delivery of high-quality rehabilitation services that meet the needs of clients. The role involves traveling within the health district and requires strong communication and interpersonal skills.

  • Key Responsibilities:
  • Deliver contemporary brain injury rehabilitation services to adults with acquired brain injuries
  • Work collaboratively with a team of senior clinicians to achieve service goals
  • Travel within the health district to conduct home visits and assessments
Requirements

To be successful in this role, you will need:

  • A degree in a relevant field (e.g. psychology, social work, occupational therapy)
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Experience working with adults with acquired brain injuries
