Responsibilities:
  • Develop and implement individualized case plans that meet the unique needs of clients
  • Collaborate with service providers to ensure seamless access to necessary resources and support

  • Establish effective relationships with key partners, including families, community members, agencies and government departments
  • Maintain accurate records and information management systems, including case files, reports and data entry

  • Support departmental objectives through active participation and contribution to governance processes
Key Skills and Qualifications:
  • Proven ability to work effectively with diverse populations, including youth, families and community groups
  • Strong communication and interpersonal skills, with ability to negotiate and collaborate with various stakeholders
  • Ability to maintain confidentiality and handle sensitive information with discretion
  • Highly developed problem-solving and analytical skills, with capacity to think critically and creatively
  • Capacity to work independently and as part of a team, with ability to adapt to changing priorities and circumstances
