Job Summary

We are seeking a motivated and compassionate Care Coordinator to join our team. As a key member of our Disability Support Team, you will play a vital role in delivering high-quality support services to individuals with disabilities.

This exciting opportunity offers great work-life balance, flexibility, and the chance to make a real difference in the lives of others. You will have the autonomy to pick shifts ahead of time using our Self-Rostering app, ensuring your schedule aligns with your needs.

Key Responsibilities

  • Deliver individualized support plans tailored to meet the unique needs of clients
  • Provide emotional support and companionship to enhance the overall well-being of clients
  • Maintain accurate records of client interactions and progress
  • Collaborate with multidisciplinary teams to ensure seamless service delivery

Required Skills and Qualifications

To be successful in this role, you will require:

  • A Certificate III or IV in Disability or related field
  • Experience working with individuals with disabilities
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and discretion
