Discharge Liaison Coordinator

We are seeking a highly skilled and experienced Discharge Liaison Coordinator to join our Access and Flow team at the Women's and Children's Health Network. As a key member of our team, you will play a critical role in ensuring the smooth transition of patients with complex health needs from our hospital to their communities.
Key Responsibilities- Collaborate with multidisciplinary staff to advise and coordinate potential and pending transfers, ensuring seamless communication and coordination with country clients, families, and health professionals.
- Consult with key stakeholders to implement and coordinate a comprehensive statewide plan for effective service delivery and efficient management of children and adolescents with complex high health needs.
- Develop and maintain sound systems and processes, review and develop policies and procedures, pathways, and projects to enable efficient and effective discharge planning across the Women's and Children's Health Network.
- Registered or eligible for registration as a Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse and/or Midwife with at least 3 years post-registration experience.
- Demonstrated competence in the relevant area of nursing and/or midwifery practice in accordance with the relevant standards.
- Effective leadership skills, including highly developed skills in communication, problem-solving, conflict resolution, and negotiation.
- Experience in discharge planning and assisting patients in the transition to home.
- Ability to work effectively within a multidisciplinary team.
