Case Worker Job Summary

This is a challenging and rewarding role where you will provide AOD counselling and intensive case work to individuals and families experiencing the impact of addiction.

The successful applicant will have strong interpersonal skills, be able to work effectively with other members of a multi-disciplinary team and possess a good working knowledge of AOD referral and support options.

Key Responsibilities:
  • To facilitate the assessment, intake, counselling, referral, educational/recreational support and case management activities for clients/residents of the centre.
  • To develop and monitor Individual Treatment Plans and maintain case management documentation, records, reports and data collection.
  • Acquire and maintain a good working knowledge of AOD referral and support options for residents within TSA Network and external agencies.
Requirements:
  • Tertiary or other qualifications within Community Services, Social Work or similar discipline, or extensive relevant experience with a minimum of Certificate IV qualifications in AOD.
  • Current Driver's licence.
  • Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients.
