Youth Health Care Coordinator

We are seeking a skilled professional to fill the role of Youth Health ED Navigator. This is a full-time, on-site position based in Sydney, NSW.
Key Responsibilities- The Youth Health ED Navigator will provide direct support to adolescents and young adults accessing emergency department services.
- This includes conducting assessments, developing care plans, collaborating with multidisciplinary teams, providing health education, and connecting clients with relevant community resources and follow-up services.
To be successful in this role, you will have strong clinical skills, excellent communication and interpersonal skills, and the ability to work effectively within a team environment.
Essential Skills and Qualifications- Clinical skills in assessment, care planning, and case management
- Strong communication, interpersonal, and advocacy skills
- Ability to work effectively within multidisciplinary teams
- Knowledge of youth health issues and mental health first aid
- Experience in providing health education and promotion
- Proficiency in documentation and record-keeping
