Job Description:

The primary responsibility of a Clinical Aged Care Assessor is to conduct comprehensive assessments of individuals requiring care and support. This involves evaluating the physical, emotional, and social needs of clients to develop personalized care plans that cater to their unique requirements.

  • Conduct thorough client assessments to identify their care needs and preferences.
  • Develop and implement individualized care plans that prioritize client autonomy and well-being.
  • Maintain accurate and up-to-date records of client assessments, care plans, and progress.
  • Collaborate with multidisciplinary teams to ensure seamless service delivery and high-quality client outcomes.

Required Skills and Qualifications:

To excel as a Clinical Aged Care Assessor, you will require:

  • Tertiary qualification in a health-related discipline (e.g., nursing, occupational therapy, social work).
  • Current registration with relevant professional associations (e.g., Australian Health Practitioners Regulation Agency).
  • Proven experience in delivering aged care services, preferably in residential or community settings.
  • Excellent communication, interpersonal, and time management skills.
