Job Description:

We are seeking a highly skilled Aged Care Clinical Assessor to join our team. As an Aged Care Clinical Assessor, you will play a crucial role in identifying the needs of older adults and connecting them with services that promote their independence and well-being.

Your key responsibilities will include conducting thorough assessments to identify the needs of older adults, facilitating referrals to essential services, promoting functionality, and engaging with stakeholders such as families and carers.

In this role, you will have the opportunity to make a real difference in the lives of older Australians, working in a supportive and collaborative environment.

Required Skills and Qualifications:
  • At least 1 year of experience in aged care, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
  • Australian driver's licence and ability to travel within the designated service area.
