Clinical Care Coordinator

We are seeking a highly skilled and experienced Clinical Care Coordinator to join our team at Calvary Lakes Entrance. As a key member of our leadership team, you will be responsible for leading both the clinical and care teams across the home, ensuring the needs of each resident are met and the team understands their identity, culture, diversity, beliefs, and life experiences.
Key Responsibilities- Lead the clinical and care teams to ensure high-quality care and services are delivered to residents.
- Collaborate with the Home Manager to develop and implement care plans that meet the needs of residents.
- Ensure effective communication and coordination between teams to ensure seamless care delivery.
- Develop and maintain relationships with residents, families, and external stakeholders to ensure a positive and supportive environment.
- Manage human resource issues and provide guidance and support to team members as needed.
- Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum of 3 years' experience in a management role within the aged care sector.
- High level of commitment, integrity, honesty, and trustworthiness.
- Demonstrated knowledge and understanding of the aged care sector and ability to identify changing trends and best practices.
- Effective listener with excellent communication and presentation skills.
- Ability to understand and use information technology and programs such as Word, Excel, etc.
- Current National police check.
