The Clinical Care Coordinator is a key leadership position responsible for overseeing the clinical and care teams across the home. In collaboration with the Home Manager, the successful candidate will ensure that the needs of each resident are met and that the team is equipped to provide high-quality care.
Key Responsibilities- Lead the clinical and care teams to deliver person-centered care that meets the unique needs of each resident.
- Collaborate with the Home Manager to develop and implement care plans that promote resident well-being and quality of life.
- Ensure that the team is equipped with the necessary skills and knowledge to provide high-quality care, including infection control and WH&S practices.
- Manage human resource issues, including recruitment, training, and performance management.
- Develop and maintain relationships with external stakeholders, including families and healthcare providers.
- Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- A minimum of 3 years' experience in a management role within the aged care sector, with a proven track record of success.
- High level of commitment, integrity, honesty, and trustworthiness.
- Demonstrated knowledge and understanding of the aged care sector, including current trends and best practices.
- Effective communication and interpersonal skills, with the ability to communicate at all levels.
- Ability to manage human resource issues, including recruitment, training, and performance management.
- Working knowledge of WH&S and Infection Control practices.
