About the Role

**Clinical Care Coordinator - Aged Care Leadership Role**

We are seeking a highly skilled and experienced Clinical Care Coordinator to lead our clinical and care teams in our aged care facility. As a key member of our leadership team, you will be responsible for ensuring the needs of our residents are met and that our teams are equipped to provide high-quality care.

Key Responsibilities
  • Lead the clinical and care teams to ensure the needs of residents are met
  • Collaborate with the Home Manager to develop and implement care plans
  • Ensure compliance with aged care regulations and standards
  • Develop and maintain relationships with residents, families, and healthcare professionals
  • Provide guidance and support to team members to enhance their skills and knowledge
Requirements
  • Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Minimum of 3 years' experience in a management role within the aged care sector
  • High level of commitment, integrity, honesty, and trustworthiness
  • Demonstrated knowledge and understanding of the aged care sector
  • Effective communication and interpersonal skills
  • Ability to manage human resource issues and negotiate effectively
  • Working knowledge of WH&S and Infection Control
