We are seeking a highly skilled and compassionate individual to join our team as a Community Support Worker. This rewarding role involves empowering individuals with disabilities to live in the least restrictive environment and maintain their quality of life.

Responsibilities:
  • Develop and implement future goal plans and support plans for clients in collaboration with Case Managers.
  • Assist individuals in maintaining their health and wellbeing through regular check-ins and support.
  • Effectively communicate with stakeholders, including family members, carers, and healthcare professionals.
  • Contribute to continuous quality improvements by identifying areas for growth and development within the team.
  • Promote positive behaviours and independence in a supportive environment, encouraging individuals to take control of their lives.
About You

To excel in this role, you should possess the following qualities and qualifications:

  • Effective and professional interpersonal and communication skills.
  • Accurate and timely decision-making abilities.
  • The ability to thrive in a fast-paced environment.
  • Relevant qualifications or studying towards them.
  • A current Australian driver's license.
  • Current Blue Card or willingness to obtain one.
  • Familiarity with the Disability Services Act 2006 and screening requirements.
