Overview
Supported Living Facilitator role at UnitingCare Queensland (Disability Services, NDIS).
Your role
Work in partnership with the person being supported, their family and informal support networks to help the person achieve their hoped-for outcomes.
Provide support in everyday living situations, including personal care as identified, with an emphasis on choice and quality of life.
Participate in the development, implementation and review of individual plans that focus on the person's life goals.
Enable people to attain personal autonomy over their own lives and the services they receive.
Uphold the right to respect, privacy, dignity and confidentiality for the person being supported and their family.

What we're looking for
Able to work effectively in a team-oriented environment and provide excellent support to clients and coworkers.
Flexible and reliable, with the ability to work varied hours and days.
Strong spoken and written English communication skills.
Experience in Disability, Aged and Community sectors is ideal but not essential; positive attitude and willingness to learn are key.
Current NDIS Worker Screening check and Blue Card required, assistance available to apply prior to employment.
Open Driver's License.
Current CPR and First Aid Certificate (required before starting).
