Job Description

As a Supported Living Facilitator, you will play a pivotal role in enhancing the lives of individuals with disabilities by providing personalized care and support services. This entails assisting clients with daily living activities, promoting their independence, and fostering a sense of community.

Key Responsibilities:
  • Deliver exceptional care and support to clients residing in the community, encompassing personal care, daily living assistance, and transportation.
  • Collaborate effectively with clients, families, and other professionals to develop and implement tailored care plans.
  • Evaluate and ensure that all care and support services align with client needs and preferences.
Requirements:
  • Able to work harmoniously within a team-oriented environment and provide outstanding support to clients and colleagues.
  • Effective communication skills in spoken and written English.
  • Prior experience working in the Disability sector is advantageous but not mandatory – a positive attitude and willingness to learn are essential.
  • A valid NDIS Worker screening check.
  • Current CPR and First Aid certification.
  • Valid driver's license and access to a vehicle.
