We are seeking a skilled Social Support Specialist to join our team, providing person-centered support services to individuals and families at risk of homelessness.

\

This role involves developing and implementing case plans using a Trauma-Informed approach, securing and maintaining housing, promoting independence and community connection, and maintaining accurate client records.

\

About You:

  • \
  • Proven experience in case management and assessment, particularly within a Trauma Informed Framework\
  • Familiarity with early intervention/prevention strategies, including support for families, individuals at risk of homelessness, and vulnerable populations\
  • Knowledge of the public and community housing sector, with an understanding of housing, mental health, substance abuse, and domestic violence issues\
  • Excellent written and verbal communication skills, with the ability to negotiate and influence effectively\
  • Collaborative mindset with the ability to build strong relationships with diverse stakeholders\
