Job Title:

Nurse Case Manager

About the Role:

We are seeking a skilled Nurse Case Manager to join our team. In this role, you will be responsible for coordinating patient care from acute care through the continuum.

Responsibilities:
  • Develop, implement, and evaluate individualized patient plans based on referrals from nursing, physician, and interdisciplinary rounds.
  • Collaborate with patients, families, physicians, and interdisciplinary teams to ensure seamless care transitions between different levels of care.
  • Monitor patient hospitalization and promote effective healthcare resource utilization.
  • Coordinate utilization review activities for assigned patients, focusing on quality, cost-effective, and medically necessary care in the appropriate setting.
Requirements:
  • Bachelor's degree in Nursing (BSN)
  • Two years of recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management
  • Demonstrated ability to communicate clearly and sensitively in writing and orally with physicians, patients, and families, health care professionals, and others
Benefits:

Work Environment:

This is a part-time position with a 20-hour week schedule, Monday-Friday, with flexibility to float as needed.
