Job Overview

As a Community Mental Health Clinician, you will provide crisis assessments, interventions, counselling and psychoeducation to individuals in acute care services. This role involves working collaboratively with internal teams and external partners to ensure seamless, coordinated care.

Key Responsibilities
  • Conduct crisis assessments and interventions for individuals in acute care services
  • Develop and implement individualized plans for consumers, taking into account their specific needs and circumstances
  • Collaborate with internal teams and external partners to ensure comprehensive, person-centered care
  • Support the development of strong partnerships across services, promoting wraparound support for complex caseloads
About You
  • You hold a degree in Social Work and are eligible for membership of the Australian Association of Social Workers (AASW)
  • Have experience in community mental health, including acute and triage settings, risk assessment, and crisis intervention
  • Are committed to holistic, person-centered practice that includes families and carers as part of the care team
