This role provides clinical leadership within a multidisciplinary team. As an Integrated Care Coordinator, you will provide expert clinical skills and knowledge to support care coordination for consumers.
Key Responsibilities:- Provide clinical leadership within the team.
- Expert clinical skills and knowledge to members of the team.
- Care coordination support to consumers.
- Registered Nurse (Division 1) with AHPRA registration.
- Post-registration qualifications in Mental Health and at least 3 years' experience working in Mental Health.
- Demonstrated skills in mental health assessments and interventions.
- Ability to provide leadership to a multidisciplinary team across services and settings.
- Demonstrated organisational skills and ability to manage a complex caseload with competing priorities.
