Job Title: Community Care Worker

We are seeking a compassionate and skilled Community Care Worker to join our team in delivering high-quality support services to individuals of all ages and abilities.

Job Description:

The primary objective of this position is to provide person-centred care that promotes wellness, independence, and autonomy. This will involve supporting individuals with daily living activities, personal care, and domestic duties, as well as providing respite support and promoting social inclusion.

Key Responsibilities:

  • Deliver high-quality care and support to individuals in their homes or community settings
  • Develop and implement personalized care plans that cater to the unique needs and goals of each individual
  • Maintain accurate records and reports, as required
  • Collaborate with other healthcare professionals and stakeholders to ensure seamless service delivery

Required Skills and Qualifications:

To be successful in this role, you will require:

  • A qualification in aged care, disability, or a related field
  • Experience working with vulnerable populations, preferably in a community care setting
  • Excellent communication and interpersonal skills
  • A willingness to work on a casual basis initially, with potential for permanent part-time employment after 6 months
