Traditional Home Health Care is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality, non-medical care to our clients in their homes.

Key Responsibilities:
  • Coordinate client services and manage scheduling issues
  • Build lasting relationships with clients, referral sources, payers, and community organizations
  • Provide superior customer service and quality home care
  • Manage and grow your client caseload
  • Develop strong, communicative relationships with the team
  • Interview, hire, and orient reliable and compassionate caregivers
  • Function as primary support for daily EVV and payroll functions
Requirements:
  • Previous home health care or home care experience highly desirable
  • High School or GED required. Associate's Degree preferred
  • Staffing experience preferred
  • Excellent verbal/written communication, time management and organizational skills
  • Ability to effectively multi-task
  • Effective problem solving skills
  • Strong presentation and training skills
  • Strong customer service skills
