Community Care Specialist

A key role where you will provide a client-facing and referral service conducting assessments and creating support plans to register new clients in a timely manner.

You will work closely with the scheduling officer and services team to ensure clients receive the necessary care and support. This involves identifying which days and times have the most availability to commence new clients, ensuring clients requiring multiple supports receive priority.

About the Role:

Key responsibilities include:

Assessment and Support Planning:

  • Conduct client service referral intake process, onsite assessments, re-assessments, and reviews of clients' needs for care services.
  • Develop Support Plans from onsite assessments, matching client preferences with available rosters.
  • Manage time between Home Care Packages and Commonwealth Home Support Packages, re-assessments, and new commencements.

Client Engagement:

  • Promote In-Home Aged Care services to prospective clients, highlighting personal care, domestic assistance, and social support.
  • Liaise with Client Service Coordinators and Rostering teams to ensure timely service provision and accurate record-keeping.

Accountability:

  • Display informed affinity with our vision, values, and mission, helping achieve strategic plans and business objectives.

This position requires strong communication and organizational skills, with a focus on delivering high-quality client outcomes.

The ideal candidate will be able to work independently and as part of a team, with a commitment to providing excellent customer service and promoting our In-Home Aged Care services to the community.
