Job Description

Deivian Care Services is seeking a Support Worker to join our team. The ideal candidate will have expertise in helping vulnerable and marginalized older persons through high-quality case management, social work, and outreach.

You will play an important role for many participants who may be excluded from mainstream healthcare, social support, and opportunities, negatively impacting their functioning and independence in the community. You'll use evidence-based, integrated, and person-centered approaches to give individualized assistance using clinical frameworks and modalities to create co-developed realistic recovery programs that are both individualized and powerful.

Required Skills & Qualifications
  • A person-centered, values-based approach to service delivery informed by a proven commitment to social justice concerns.
  • Knowledge and understanding of trauma-informed, strengths-based and person-centered care.
  • Demonstrated communication and relationship management abilities, as well as the ability to maintain collaborative working relationships with a wide range of stakeholders.
