
Nurse Care Manager

As an On-Call Nurse with Hospice, you will have the autonomy to utilize your nursing skills to positively impact the experiences of patients and families. You will be relied upon to take care of patients and families who need help after hours in typically urgent times.
You will be invited into patients' homes and lives during uncertain times and have the honor of becoming a trusted face. For many team members, this is more than a career - it's a calling.
Key Responsibilities- Assess physical, psychosocial and spiritual needs of the patient and family.
- Provide intermittent skilled nursing services to the patient under the patient's physician's direction.
- Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
- Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
- Provide emotional support to the patient and family.
- Refer patients and/or family members to other services when appropriate.
- Provide instruction (health care teaching) to the patient and family as related to care in a residence.
- Make visits to patient's residences at the time of death.
- Maintain, promptly complete, and return all documentation as required.
- Communicate and act as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
- Relay pertinent information to appropriate team members.
- Participate in meetings as initiated by team leader.
- High School Diploma or equivalent required.
- Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
- Licensed RN/LPN in state of Ohio.
- Experience preferred in pain and symptom management, oncology, or mental health.
- Ability to communicate effectively, orally and in writing.
- Ability to work independently and collaboratively with a team.
- Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
- Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
- Ability to access patients and families in any residential and/or public setting in any weather condition.
- Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, donor database, grant portals and virtual meeting technology.
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to Hospice.
