Hospice Care Nurse
The RN Case Manager plays a pivotal role in ensuring the delivery of quality hospice care to patients and their families. With a strong focus on customer service, innovation, and integrity, this position requires a compassionate and skilled nurse who can make a meaningful difference in the lives of others.
Key Responsibilities:- Deliver ongoing assessment and nursing care to patients in their homes and healthcare facilities.
- Develop and implement comprehensive care plans that address current needs and ensure compliance with federal, state, and local regulations.
- Communicate effectively with other healthcare providers about changes in condition and patient needs.
- Assess patient and caregiver needs and provide education when necessary.
- Oversee the implementation of the patient's Plan of Care and evaluate its effectiveness on an ongoing basis.
- A valid license for the state of Ohio to practice as a Registered Nurse (RN) is required.
- One year experience in acute care or medical-surgical nursing is required; experience in hospice is preferred.
- Proficiency with Windows, Microsoft Office, and the internet is required; proficiency with electronic medical records (EMR) is preferred.
- Able to read, write, speak, and understand the English language.
