Key to our success is the role of the Case Manager, an integral part of our Intensive Family Preservation Program. As a dedicated professional, you will play a vital role in delivering client-focused and outcome-based services.

About the Role
  • You will work collaboratively with clients who experience complex issues, focusing on reducing risks and creating positive change within their home environments.

Responsibilities include:

  • Advocating for the needs of Aboriginal people and contributing to cultural case plans.
  • Implementing trauma-informed care under the Sanctuary model, integrating it into daily work.
  • Fostering respectful relationships with stakeholders, government departments, and the community for seamless service delivery.
  • Developing and implementing case plans in collaboration with families and other service providers, conducting in-home visits to support the achievement of agreed goals.
  • Ensuring that the voices of clients and families are heard in case planning and goal setting.
  • Working respectfully to ensure that case plan goals are achievable and realistic.
  • Supporting parents and children attending playgroup.

Required Skills and Qualifications:

  • Relevant qualifications in Human Services (Diploma of Community Services or equivalent; Degree qualifications preferred).
  • Practical experience working with vulnerable families and identifying at-risk children.
  • Demonstrated expertise in client-centered case management.
  • Knowledge of the Community Services sector, trauma, crisis impacts on families, child protection issues, and legislation.
