Primary Care Professional Opportunity

We are seeking a qualified Primary Care Professional to deliver outreach services in a waterfront community near Victoria. The successful candidate will provide accessible primary episodic care services addressing immediate medical needs and work towards decreasing barriers and social determinants of health in accessing care.

Key Responsibilities:
  • Deliver outreach primary care and episodic medical services to clients at the Sooke Food Bank and Sooke Shelter.
  • Provide both OAT (opioid agonist treatment) and primary care services to a broad range of clients.
  • Collaborate with a primary care RN, an MHSU RN, and a Medical Office Assistant (MOA) as part of the care team.
Supporting Team:
  • The successful candidate will be supported by a primary care RN, an MHSU RN, and a MOA.

This role requires providing primary care services to adults and children impacted by poverty, colonization, stigma, and homelessness. The ideal candidate will possess excellent communication and interpersonal skills to work effectively with diverse populations.

Job Requirements:
  • Apart from delivering primary care services, the successful candidate will also need to maintain accurate records of patient information.
  • Foster strong relationships with community partners to ensure seamless service delivery.
