Care Coordinator Role

This is an opportunity to make a positive impact on the lives of individuals with complex mental health needs.

The role involves working collaboratively with multidisciplinary teams, consumers, carers and other key stakeholders to coordinate care and support services that enable safe transitions from hospital to community settings.

As a Care Coordinator, you will be responsible for assessing and progressing the care plans of consumers, promoting effective participation in mental health treatment, support and recovery.

You will work closely with existing inpatient and community teams to ensure seamless delivery of evidence-based, strengths-focused, recovery-oriented assessment and treatment.

Key Responsibilities
  • Coordinate care and support services for consumers with complex mental health needs
  • Assess and progress care plans to promote safe transitions from hospital to community settings
  • Work collaboratively with multidisciplinary teams, consumers, carers and other key stakeholders
  • Promote effective participation in mental health treatment, support and recovery
Required Skills and Qualifications

To be successful in this role, you will need:

  • A relevant tertiary qualification and current registration with the relevant professional body
  • Experience working in mental health, preferably in a coordination or case management role
  • Strong communication and interpersonal skills
  • Ability to work collaboratively with diverse stakeholders
