Support Coordinator

Role Overview

This role involves providing personalized care and support to clients, empowering them to live safely and independently in their own homes.


Key Responsibilities
  • Deliver high-quality personal care services, including showering, dressing, grooming, and continence support, while maintaining the dignity and comfort of clients.
  • Assist clients with medication management, ensuring they receive the correct dosage.
  • Offer daily living support by helping high-care clients with activities that make everyday life easier.
  • Lend a hand with domestic duties, light cleaning, laundry, meal preparation, and keeping living spaces safe and welcoming.
  • Provide social support and companionship, including driving clients to shops, appointments, or community activities.
  • Fully understand and adhere to individualized care plans to meet each client's unique needs and preferences.
  • Encourage independence by supporting clients to do as much as they can for themselves.

Requirements
  • Aged care or community care experience, preferably with a focus on personal care (showering, dressing, continence support).
  • Certificate III in Aged Care (or equivalent) is highly desirable.
  • A current Australian driver's license and access to a reliable vehicle.
  • A genuine passion for supporting older people to live independently in their own homes.
  • The ability to build positive, respectful relationships with clients and their families.
  • Good communication skills and a caring, patient nature.
