Role Overview

We are seeking a dedicated and compassionate individual to join our team as a Case Manager for our Supported Housing Program.

The successful candidate will provide specialist support to people who are homeless or at risk of homelessness or in housing need, helping them navigate challenging situations and achieve their goals.

  • Provide holistic support to individuals and families experiencing homelessness or housing instability.
  • Work collaboratively with internal stakeholders to develop and implement effective case management plans.
  • Maintain accurate and up-to-date records of client interactions and progress.
