About the Role:

We are seeking a compassionate and collaborative professional to deliver coordinated support to clients engaged in the WARATAH Program.

Main Responsibilities:
  • Implement care plans and connect clients to essential health and social services.
  • Craft personalised support plans with clients to enhance health and well-being.
  • Address broader challenges affecting health, including financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.

To be successful in this role, you will require:

  • Relevant tertiary qualifications in Nursing, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
  • A solid understanding of the broader determinants of health, including social, economic, and environmental factors.
  • Proven experience in co-developing person-centred support plans prioritising client well-being and aspirations.
  • Advocacy skills to enhance client access to programs and inspire community participation.
  • Exceptional communication and interpersonal abilities, particularly with culturally and linguistically diverse backgrounds and First Nations communities.
