Health Support Specialist

We are seeking a highly skilled and compassionate Health Support Specialist to join our collaborative team. As a key member of our team, you will play a vital role in delivering holistic support to individuals engaged in our programs.

Your primary responsibility will be to provide coordinated support to clients, implementing personalized care plans that enhance their health and well-being. You will work closely with clients to craft tailored support plans that connect them to vital health and social services, re-establishing connections to community groups and essential services.

In addition to your clinical expertise, you will also provide guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices. Your advocacy skills will enable you to inspire community participation and enhance client access to programs.

Requirements:
  • A relevant tertiary qualification in Nursing, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
  • Proven experience in co-developing person-centred support plans that prioritise client well-being and aspirations.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with clients, colleagues, and community stakeholders.
