Care Manager Position

We are seeking a skilled and experienced Care Manager to lead our clinical and support teams in delivering high-quality care to residents. This is an exciting opportunity for a mid-senior level professional to provide clinical leadership and support, ensuring seamless care delivery and effective communication.

Key Responsibilities:
  • Assess and Develop Individualised Care Plans: Use your comprehensive clinical knowledge and understanding of the aging process to develop individualised care plans that meet the unique needs and preferences of each resident.
  • Lead and Coordinate Care Programs: Lead and coordinate the development and implementation of care programs and services, collaborating with multidisciplinary teams to ensure their success.
  • Maintain Quality Standards: Work closely with all policies and procedures, including IPC, to ensure compliance with quality standards and deliver high-quality care to residents.
Requirements:
  • Registration as a Registered Nurse (AHPRA): You must hold a current registration as a Registered Nurse with AHPRA.
  • Aged Care Legislation and Quality Standards: Have a thorough understanding of the Aged Care Legislation and Aged Care Quality Standards.
  • AN-ACC Assessment Processes: Demonstrate knowledge and understanding of AN-ACC assessment processes.
  • Leadership and Coaching Skills: Possess excellent leadership and coaching skills, with the ability to influence others and drive results.
