
Senior Care Coordinator

This is a leadership position where you will have the scope to own one house end-to-end, focusing on providing exceptional care and support to our participants.
We are looking for an experienced Support Worker who is ready to step into their first Team Leader role, with a passion for leading teams and making a difference in the lives of people with disabilities.
- As a successful candidate, you will be responsible for owning participant documentation, including risk assessments and participant profiles.
- You will provide guidance and decision-making for urgent issues as part of our on-call lead structure.
- Our team relies on your ability to coordinate client care, liaising with healthcare providers and allied health professionals.
- You will also run team meetings, set agendas, track actions, and maintain momentum.
- Support worker management is another key aspect of this role, involving regular check-ins, coaching performance, and upholding practice standards.
- Leave and administration tasks include processing leave requests, approving timesheets, and keeping records tidy.
- The house's upkeep falls under your responsibility, overseeing maintenance, and escalating repairs promptly.
- You will recruit and onboard new staff, manage buddy shifts, and ensure confident handovers to the roster.
- Capture work by completing meeting minutes for all stakeholder meetings and circulating actions.
- Finally, lead rostering to ensure skill-matched, person-centred shifts.
About This Role:
- We are seeking experience in disability or community services, ideally in a house or SIL setting.
- Candidates should possess calmness, organisation, and proactivity, along with comfort making decisions on-call.
- A strong communicator can coach, give feedback, and bring teams together.
- Paperwork compliance and updating skills are essential.
- Tech proficiency with rostering/timesheet systems and basic Office/Google tools is required.
- A current Driver's Licence and reliable transport are also necessary.
