About the Role

The Child Health Outreach Program (CHOP) Team Leader is responsible for providing management and coordination of the CHOP, ensuring cultural integrity and high standards of clinical care and case management. The position has a strong focus on outreach work with clinical, liaison and administrative responsibilities.

Key Responsibilities

  • Lead and manage a multidisciplinary team to provide comprehensive child health services to remote communities
  • Develop and implement culturally sensitive programs to improve health outcomes for children and families
  • Provide clinical supervision and support to staff to ensure high-quality patient care
  • Collaborate with other healthcare providers to ensure seamless service delivery and referrals
  • Manage budgets and resources to optimize program effectiveness

Requirements

  • Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
  • At least five years clinical experience in community-based child health nursing, case management and chronic disease management
  • Demonstrated ability to lead and manage a multidisciplinary team
  • Strong understanding of Aboriginal culture and experience working with Indigenous communities
