Be a Youth Mental Health Care Coordinator">

This role is part of a multidisciplinary team providing mental health care coordination to young people aged 12-25 years and their families.

As a Clinical Care Coordinator, you will work with young people who present with diverse mental health concerns and support them in developing skills for independent living, budgeting, building social connections, and accessing community organizations.

">

You will have the opportunity to engage with a wide range of stakeholders, including young people, their families, friends, carers, and other agencies, and foster sound relationships with people at all levels.

This position requires travel, telehealth, home visits, and overnight stays. Flexibility in hours is necessary for this role.

Required Skills and Qualifications:

">
  • ">
  • Qualifications in Psychology, Social Work, or related disciplines with a minimum three-year undergraduate degree in medical or allied health disciplines and postgraduate experience in the field demonstrating the ability to support young people.">
  • Demonstrated skills and experience in monitoring risk, managing complex presentations, and coordinating care for young people.">
  • Exceptional communication skills and the ability to engage and communicate with groups of people, tailoring communication to suit a specific target group or age range.">
  • Experience in delivering skills for independent living, budgeting, building social connections, and increasing access to community organizations.">
  • Desirable: Knowledge of local youth/health sectors, knowledge of Youth Enhanced Service Model (Orygen), and an understanding of trauma-informed practice.">
  • Ability to participate in, develop, and maintain productive and sustainable community and professional partnerships to enhance youth health outcomes.">
