Senior Clinical Care Coordinator

We are seeking an experienced and passionate Registered Nurse to work in a variety of aged care facilities across Australia.
About the RoleThis is a fantastic opportunity for a Registered Nurse to take on a leadership role and make a real difference in the lives of our residents. As a Senior Clinical Care Coordinator, you will be responsible for providing clinical guidance and support to care staff, ensuring the delivery of high-quality person-centered care.
You will lead a team to deliver exceptional care, develop and implement care plans, and collaborate with healthcare professionals to achieve optimal resident outcomes.
What You Will Be Doing- Providing clinical guidance and support to care staff
- Developing and implementing care plans that cater to individual resident needs
- Collaborating with healthcare professionals to ensure seamless care coordination
- Leading a team to deliver high-quality person-centered care
- Documenting vital signs, medication intake, and resident assessments
- Assisting with meal consumption and monitoring nutrition
- Family and resident education
- Infection control and prevention
- Supervising Allied Health Assistants (AINs) and Enrolled Nurses (EENs)

Requirements
- Current Australian Health Practitioner Regulation Agency (AHPRA) registration
- COVID-19 vaccination (including booster)
- Flu vaccine (as some facilities require it)
- Current police check and National Disability Insurance Scheme (NDIS) clearance or willingness to obtain these
- Availability to start within 4 weeks
