Catholic Healthcare seeks an experienced Registered Nurse to join our Home and Community Team. The successful candidate will provide comprehensive assessment, nursing care, and evaluation of care to enable clients with complex health needs to achieve their healthcare goals and continue living in their own home.

  • Deliver direct clinical nursing care to clients in their homes, adhering to best practice nursing procedures.
  • Conduct clinical assessments, dressings, medication assessments, and catheter changes, as required.
  • Develop holistic nursing care plans in collaboration with clients, carers, Care Advisors, and Allied Health professionals, where relevant.
  • Provide safe, quality nursing practice to clients with specific needs or complex needs, such as wound care, palliative care, education, and support.
  • Liaise with parties involved in client care to maintain holistic healthcare and wellbeing.
  • Ensure accurate and timely documentation of client care.
  • Work safely to protect yourself, colleagues, and clients, adhering to all policies and procedures.

Requirements

  • A kind heart and genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar.
  • Minimum 3 years postgraduate nursing experience.
  • Experience in clinical assessments, including PAS and RUDAS.
  • Strong communication skills and a positive attitude.
  • Sound computer skills, particularly with Microsoft Office products, and the ability to learn new systems.
  • Driver's license and a reliable vehicle.
  • Willingness to apply for a Police Check and NDIS clearance.
