Clinical Aged Healthcare Specialist

**Job Overview**
">Clinical Aged Care Assessor role involves joining a growing team to deliver needs-based assessments. You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care Act.
This position requires collaboration with clients, families, and multidisciplinary teams as required. You will also work collaboratively with colleagues to promote a supportive and positive team culture.
">Key Responsibilities:

- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services">
- Reviewing support plans to meet client's changing support needs">
- Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes">
- Maintaining currency of knowledge relating to aged care services both funded and non-funded">
Requirements:
">To be successful in this role, you will need a Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area. You must also have current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA). An Australian Driver's license and access to a registered, insured vehicle are also essential.
A minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to aged persons including to people living with dementia is necessary.
