Overview
Clinical Aged Care Assessor - Mid North Coast role at Catholic Healthcare. Location: Mid North Coast; travel required within areas such as Taree and Forster. Flexible hours and days – Monday to Friday, part time 3 days per week; no shift work. Pay rate – $54.47 per hour + superannuation. 7 additional leave days per year. Pay less tax with Not For Profit Salary Packaging. Travel will be reimbursed at 0.88 cents per km in line with our travel policy. Fitness Passport discounted gym memberships. Paid Parental Leave and Long Service Leave after 5 years.
Aged Care Assessment Services (ACAS) operate independently of Catholic Healthcare and play a pivotal role in ensuring that Older Australians are assessed for the care and services they truly need. We are expanding our ACAS team across metropolitan and regional locations, with most roles involving local travel within each region. This role involves joining a growing team to deliver needs-based assessments.
What will your role look like?
You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care Act.
Day To Day This Includes
Collaborating with clients, families and multidisciplinary teams as required
Working collaboratively with colleagues to promote a supportive and positive team culture
Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
Reviewing support plans to meet client's changing support needs
Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
Maintaining currency of knowledge relating to aged care services both funded and non-funded
Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes
To Be Successful In This Role You'll Need
Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
An Australian Driver's license and access to a registered, insured vehicle
Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to aged persons including to people living with dementia
Willingness to apply for a Police Check
