Clinical Aged Care Assessor
Job Description:As a Clinical Aged Care Assessor, you will play a pivotal role in ensuring that older Australians receive the care and services they need. This involves conducting thorough assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services under the Aged Care Act.
Your primary responsibility will be to collaborate with clients, families, and multidisciplinary teams to deliver needs-based assessments. You will conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
In addition, you will review support plans to meet changing client needs and maintain currency of knowledge relating to aged care services.
Required Skills and Qualifications:- Tertiary qualification in a health-related discipline directly related to health, aged care, or a related specialist area
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- Australian Driver's license and access to a registered, insured vehicle
- Minimum of one year demonstrated experience in Australia or overseas delivering services in aged care settings and/or to aged persons, including those living with dementia
