Job Description

We are seeking a dedicated and compassionate Clinical Aged Care Professional to join our team. The successful candidate will be responsible for conducting assessments to identify the complex needs of clients and referring them to appropriate aged care services under the Aged Care Act.

The role involves collaborating with clients, families, and multidisciplinary teams to deliver needs-based assessments. Key responsibilities include:

  • Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
  • Reviewing support plans to meet client's changing support needs
  • Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
Requirements

To be successful in this role, you will need:

  • Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • An Australian Driver's license and access to a registered, insured vehicle
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to aged persons including to people living with dementia
