Job Overview:

The Community Housing Officer role requires a dedicated professional to deliver holistic, person-centred support to clients in crisis. This position involves working collaboratively with the Emergency Housing team to provide immediate crisis support and connect clients with relevant external services.

Key Responsibilities:

  • Conduct comprehensive initial assessments to identify client needs and develop individualised case plans.
  • Monitor and adjust case plans to ensure they remain effective and responsive to client progress and evolving needs.
  • Provide timely crisis intervention and support, connecting clients with specialist services as required.
  • Collaborate with real estate professionals to secure stable housing outcomes for clients.
  • Manage a portfolio of ten residential properties, overseeing regular inspections and tenancy transitions.
  • Liaise with key stakeholders, including Police, Child Safety, Corrections, Youth Justice, Public Guardian, and educational institutions.
  • Deliver coordinated, wraparound services to promote client wellbeing and long-term stability.
  • Promote independent living skills, including budgeting, daily routines, and shared responsibilities.
  • Exercise sound judgment when determining whether to escalate matters to specialist services.

Required Skills and Qualifications:

  • Qualifications in Mental Health, Social Work, or Community Services (highly regarded).
  • Experience working with individuals experiencing or at risk of homelessness.
  • Strong understanding of complex needs.
  • Proven experience in case management, particularly with high-needs or vulnerable populations.
