Care Coordinator Role

As a Care Coordinator, you will play a vital role in supporting individuals living with dementia in social settings. This position involves coordinating individualized support and services for clients from onboarding through to review, ensuring their needs and goals are met throughout their dementia journey.

Key Responsibilities
  1. Monitoring contractual hours of services within allocated budgets
  2. Inputting and maintaining accurate data/records regarding staff, clients, and carers
  3. Ensuring ongoing evaluation of service delivery to identify gaps in service provision and ensure continuous improvement
  4. Assisting in the formulation and implementation of team procedures
  5. Coordinating referrals to ensure the implementation of appropriate services for people with dementia, adhering to guidelines, policies, and procedures
  6. Using a reablement and wellbeing approach to work collaboratively with clients to define their goals and encourage feedback on how they would like to shape their service
  7. Coordinating support workers, ensuring effective staff utilisation in appropriate services to achieve the best outcomes for our clients
  8. Providing dementia-specific information to clients and carers as requested
  9. Coordinating services including social support, in-home respite, groups, and day centre services
  10. Providing appropriate feedback to the House Manager on any service issues
  11. In conjunction with the House Manager, providing supervision, monitoring, support, training, and performance evaluations for staff and volunteers
  12. Assisting with recruitment and onboarding, as directed by the House Manager
  13. Processing timesheets for staff/volunteers
  14. Monitoring safe working conditions for all community and respite staff
  15. Collaborating closely with other departments to ensure best practice in our service provision
  16. Liaising with external service providers and agencies to facilitate information sharing and promotion of our services
Required Skills and Qualifications
  • Tertiary qualification in aged care or community services or extensive relevant experience
  • Previous experience in the health and aged care industry
  • Demonstrated ability to plan and organise workload to meet tight deadlines
  • Demonstrated empathy, understanding, and experience of working with people living with dementia
  • Excellent interpersonal and communication skills, including the ability to manage stakeholder relationships
  • Working knowledge of Microsoft Office applications
  • Valid driver's licence
  • NDIS Check (or willingness to obtain)
  • Up-to-date COVID-19 vaccinations
