
Care Manager for Young People

The House Coordinator is a hands-on role that involves providing case management of young people and developing strong working relationships with external stakeholders.
Key Responsibilities:- Advocate on behalf of clients to achieve identified goals and support needs.
- Develop and implement individual assessment and case plans tailored to need within a residential care setting.
- Work with support staff to develop and implement behaviour management strategies and plans for children in care.
- Respond to crisis situations, providing flexible and dynamic responses to meet the needs of children and young people.
- Provide regular opportunities for support staff to receive supervision.
This role reports directly to the Team Leader in Residential Care and oversees houses and care teams across Adelaide.
What We're Looking ForWe're seeking a people-centred individual who can connect with children and young people. You will have exceptional self-awareness, be able to handle stressful situations calmly, and practice self-care.
You will bring prior leadership experience or have worked within a coordinator or case manager position in the past. You will also have the following skills and experience:
- Bachelor qualifications in Social Work, Psychology, or related field.
- Certificate IV qualifications in one of the following: Youth Work, Community Services, Child, Youth and Family Intervention, or Disability (in conjunction with relevant experience).
- An understanding of the needs of children and young people under Guardianship of the Chief Executive.
- Experience working with vulnerable and at-risk children and young people with complex needs and/or disability.
- Demonstrated people leadership and experience in care coordination.
